Dated -08.01.2018 Gram Panchayat-Kurumpuri (Komna Block)
Visited Kurumpuri G.P. of Komna Block.Labourers complained that they are getting wages after three to Six months after works. Job cards are also not maintained by G.P. The PEO was directed to comply the matter within seven days.
Visited works like Renovation of Tank Hagrimunda at Khusdana, Renovation of Khaliabandh Tank at limdihi, Renovation of WHS Mahulmunda at Kurumpuri, Renovation of Jugalbandh tank at Daldali, Excavation of Pipalmunda tank at Latkanpada where it came to my notice through the grievance received from Basudev Dharua, Bilasini Majhi, Bishnu Dharua, Arma bewa and 28 others for which the wages of Rs 18,832/ was not received due to non-billed by the JE for Mahulmunda Tank and the same was also get duly conformed from the GRS and GS.
In addition to the above, several fake MRs have been prepared by the GRS and the concerned GS which was also proved and cross examined from the workers engaged. About 47 nos. of labourers not worked in the project are billed with Rs 2,30,522/- came to know from a petition from Bishnu Dharua and several others.
Dated -10.01.2018 Gram Panchayat-Biromal (Nuapada Block)
Conduct field visit and enquiry at Biromal GP for nonpayment of MGNREGS Labour wages. The labourers of six community works complained that they are not paid wages of the works and the GRS is deliberately involve in mischief (Mannulal Chandrakar) with Biromal Post Master which needs details enquiry by higher Officers. The BDO and APO is also not hearing their complaints. Several PMAY/IAY hose hold have not paid their installments which they have put grievance before me. In addition to that, many Community works labourers are not paid their wages. Postal Accounts are opened by the GRS whereas passbook is not hand over to labourers. All manipulation is done with the pass book which is in the custody of GRS and withdrawn money by fake withdrawal slip with the help of Post Master Biromal.

Visit to ongoing and completed works of Thikpali GP of Komna Block and attending beneficiaries.
Visited few community projects Improvement of Road from Babupali to Lakhmipadar, Improvement of Road from Babupali to Ganiary Pada, Improvement of Semilpali to Barakund Road, Renovation of Arkatamunda Tank at Thikpali, Improvement of Road from Sarlgipada to Tankapani, Play ground at Suklimundi, Improvement of Road from Suklimundi to Rohenpada.
Full and partial amount of 72 beneficiaries of 15-16 for Land Development have not been paid for which the complaints received from concerned beneficiaries Kunu sabar, Pinki Suna, Tarabati Suna, Tularam Sabar, Tula Satnami. The Sarpanch Gopinath Sabar, PEO Bhanupratap Dansena and GRS Bhupendra barik expressed that payment has not done due to some administrative dispute related to frequent transfer of PEOs and after my orders the payment can be made. The PEO and Sarpanch were asked to comply the matter within one month.
Dated -20.01.2018 Forest Range Khariar (Khariar Block)
Visit to Khariar Forest range and inspection of projects undertaken in the past years and payments disbursed to watch and ward workers are not done in time and paid in Cash.
Seen projects like “Sandhibahali Bald hill plantation, Economic plantation at Ranimunda, Ranimunda Chaura A PRF (inside Work Plan), Avenue plantation Keshrajpur Bagbahal Road, Plantation at Raja AT High School which is 2nd year, ANR Artatran pur 2nd year.Here also the same situation of watcher Ghanshyam Pradhani and watering expenses to Ram Barik by engaging tractor tanker were not been paid following to the above reason mentioned in Komna forest range inspection. Interacted with the labourers Gajanand Barik, Daitari patel, Dharam Singh Naik, Hemant Naik, Bharatdwaj Barik, Rudramadhav Naik whom not paid full wages.
In the work of Economic plantation at Rani munda, it was also observed that the labourers Raysing Majhi, Okilo Hathi, Jugeswar Kata, Jasobanto Kata, Tabul Kanta, Kirti Raj Kata, Pabitra Kata and the watcher Bira Nial were not paid their wages for which instructed the forester and Ranger to complete it immediately and comply.

During my visit to Jatgadh GP of Komna Block, the Sarpanch and PEO were present without any case record, however all case record was in the custody of the GRS Khirasindu Hans who was absent,stated by the PEO Mr. Bijay Kumar Majhi
Conduct field enquiry of work and payment made to MGNREGA workers in seven works the wages have been done partially. The undersigned has received several complaints from the labourersNakul Rana, Palsthi Rana, Santosh Rana, Duryodhan Majhi, Raj Kumar Ahir, Omkar Singh Majhi, Basant Majhi, Kahal Singh Majhi, Pitambar Pathar and others for the work done at Gadgadbahali to Chacharajharan, Imp of Road from Jatgad to Sikaripada and several other complaints related to works implemented in the year 2015-16, 2016-17. The PEO was asked to pay full payment. No display board was found in worksite. The PEO who expressed his ignorance about knowing nothing about works under his signature, was directed to comply within seven days making an enquiry by himself as he is answerable.

Visit to ongoing and completed works of Kendubahara GP of Nuapada Block and attending beneficiaries.Visited several works and interacted with the labourers Sadaram Majhi, Minaram Majhi, Babulal Khaira, Gaindram Harijan, Dallu Majhi, Bhanu Khaira, Puran Khaira, Tilak Majhi, Bisahin Rout, Kartik Khaira, Dinabandhu Khaira, Khamsingh Majhi, Khuman Majhi, Radhabai Majhi and others came to know that partial payment has been done to them.
It was also observed from my visit roaming in all villages that Non-payment of Labour wages for 15 person since 1 year for the work of “Improvement of Road from Loharapada to Chhattisgarh Border” was not made, Nearly 100s of Labourers alleged not received wages till date for the work of “Excavation of Sirigidi Tank” at Sirigidi village, Non-payment of Labour wages since 1 year for the work of “Improvement of Ghatwarin Farfaud Road” of Farfod Village.
Field Measurements are done by the GRS and sarpanch for the ongoing projects taken for the year of 2017-18. The concerned JE/GPTA is not remaining present at the time of measurement which violates the implementation of the scheme;C1 is not collected from willing labourers of Sirigidi and Farfaud villages as complained by them.The PEO was directed for release of wages within a week and comply.

Visited works like imp of Dhamanpuri to AWC building to Kurumpuri, Imp of Road from gobra to Pata pada with culvert at Tanwat, Renovation of Dhanuakona Bandh at Tanwat, Improvement of Road from Jhilmila to Dharsapada, Deepening of Dhatuchipa tank, Playground at Kholbahara , Improvement of road from Tanwat Keramunda to Bagbahali, Anganwadi Kendra to Kurumpuri road
Dated -10.02.2018 Gram Panchayat-Badi (Khariar Block)
Visit to ongoing and completed works of Badi GP of Khariar Block and attending beneficiaries.It was noticed that the PEO has not signed the MRs.
Visited works like Improvement of Nuabandh at Padampur, Improvement of Road from Sandhibahali UP School to limpada, Improvement of Road from Limpada to Sandhibahali UP School, Improvement of Road from Sandhibahali to Limpada, Improvement of Road from Junani to Buerbhatta Road, Improvement of Road from Karuamunda at Badi.
Interacted with the labourers worked in the aforesaid work and found almost all payments were released in time and works like Improvement of Road from Sandhibahali UP School to Nimpada where Roller and Watering charges seems more than expectation and the wages for distribution of Murum is found negligible and asked the PEO Sanatan Panda to give notes on the violation of labour Material ratio within a week.

Suomoto visited Motanuapada G.P. of Nuapada block and Payment of NREGA share to Rural Housing beneficiaries is pending and the PEO is not taking steps for issue Job-Cards to new applicant.PEO was directed to release full payment to beneficiaries as per works done, Issue Job-cards to applicant within 15 days.
Visited works like Arkata bandha at Kukrimundi, Renovation of Mota Nuapada Ramsagar Tank, Renovation of Piprahi Tank at Set jampani, Improvement of Road from Dhajabhata to Kukrimundi, and renovation of Mandir Bandha at Mota Nuapada.
Enquired the labourers named SantramMajhi,Kantilal Satnami, Kandi Satnami, Dhaniram Majhi and Narayan Majhi regarding wages disbursement. Received complaint GS SankarlalUjir for the Pending wages for the work of Renovation of MandirBandh tank for which complaint registered vide case No 54

Suomoto visit to Watershed GPs of Komna Block. Visited works of Lakhna,Dedenga,Mendhatal and found that partial payment of wages has been done.Seen the works of Field Bunding of Arjun Majhi, Iswar Majhi, Bhubaneshwar Bag, Madhu Majhi, Bhagat Majhi, Damrudhar Majhi, Yadav Bibhar, Kumar Majhi and Sahadeb Majhi of Mendhatal Dedfenga Villages of Lakhna GP. Works of Balram Bag, Pustam Gahir, Ramprasad Gahir, Bhuta Bag, Gangaram Nag, Lochan Gahir, Mitrabhanu Nag, Ujal Nag, Mohan Majhi, Jagobandhu Rout, Damrudhar Majhi, Tejram Nag, Ghasiram nag.
Part Payments were made due to non-availability of technical staffs for taking Geo-tagging photographs as reported by the SEW Sabar Babu. It was made convenient by involving GRS of Lakhna GP to assist the same which was instructed on spot and asked the SEW to take prompt steps for release of pending amount of labourers.

Conduct field visit and enquiry at Kotenchuan GP for MGNREGS Works Payment of wages to Rural housing Scheme is pending.PEO was directed for payment as per progress of works immediately.
Works like Jamdhara to Rampur Pada Road, Improvement of Road from Kotenchua to Analajuba, Excavation of Tank at Paraskhol, Paraskhol Khaliabandh, Renovation of Khaliabandh at Sunarital, Improvement of Road from Chelilendi to Jamdhara, Improvement of Road from Tilejhar to Analajuba.
Meet with the laborers Meghraj Sahu, Gurubaru Majhi, Sobhraj Sunani, Lalit Sahu, Sahatram Sahu whom worked in the aforesaid projects were paid their wages full.
Dated -27.02.2018 Gram Panchayat-Kendubahara (Nuapada Block)
Noticed Non-payment of Labour wages since 1 year for the work of “Improvement of Road from Loharapada to Chhattisgarh Border”. Nearly 100s of Labourers alleged not received wages till date. Sitting at Kendubahara for complaint rose about nonpayment of wages and basing on the same finalised for release of payments wages of labourers fully within 30 days as committed by the PEO, GRS and Sarpanch.

Sitting at Jatgad GP for settlement of pending wages. Payment wages of labourers were finalised in presence of GRS and labourers. In the work of Improvement of Jatgarh to Sikari Pada Road, 17 nos. of labourers for a pending payment of Rs. 53328/-, The work Gadgadbahali to Chacharajharan Road found 5 nos. of labourers for a pending payment of Rs 9504/- and for the Work of Improvement of Road from Jatgad to Bharuamunda Salepada wherean amount of Rs 2112/- pending which was finalised. The GRS, Sarpanch, PEO and APO were instructed to release the pending dues immediately and comply.
Dated -09.03.2018 Gram Panchayat-Darlipada (Komna Block)
Spot visit made to Darlipada G.P under Komna Block. Conduct field enquiry work and payment made to MGNREGA. Visited the past worksin presence of Sarpanch/PEO/GRS and Gaon Sathy. Found that Labourers have not given wages including Gaon Sathies.Moreover in the Case records, Sarpanch and PEO have not signed also Instructed to rectify the problems immediately. During visit of community works and interacted with some people. The subsequent day, some other workers Mansing Majhi, Deoki Majhi, Nirasa Majhi, Santram Ahir, Ballu Ahir, Nohar Lal Sahu and others along with few PRI Members complained regarding GS payment &pending wages related to the work of “Dummermunda Tank at Dummerpani” completed in the year 2015-16. Few other complaints also received related to the ongoing work of “Renovation of Lariapada Tank at Hathisara” registered complaint vide case No 26.

Spot visit to Samarsing GP of Komna Block for Horticulture & Panchayat Samity work. Conduct field enquiry of pending payments of MGNREGA workers.Visited works in presence of Sarpanch/PEO/GRS and found that Payment has not been made of Khairpadar to Chargharia Pada Road. Gaon sathy has not been there but amount in favour of Gaonsathy without mentioning name has been paid which is recoverable from PEO and GRS. The matter was discussed with BDO and to comply by a week.
After getting a complaint of Indra Rona that the wages are not depositing in his account since initiation, enquired the matter and found that an amount of Rs 4500/- was found for the particular work, deposited in the account of the relative of Laxman Rona (Ex GRS) and ordered him to return immediately to avoid further allegation.Instructed the PEO to rectify the account to prevent further misuse.Visited works like Mohulmunda Tank, Tipimunda Tank, Improvement of Road from Tengnabasa to Chindpani, Improvement of Road from Khairpadar to Chargharia Pada

Sitting at Darlipada GP Office for Case Record & Muster Roll verification of works done of Komna Block.The problems of the works visited on 09.03.18 could not be solved and a meeting conducted to day in Darlipada and Hatisara villages enquired the facts and found
regarding misappropriation of the work “Renovation of Lariapada Tank at Hatisara of DarliPada G.P under Komna Block, Nuapada. Work code of the same work 2428002004/WH/3091172 noticed, which is ongoing where in 97 nos. of labourers were engaged as per M.Rs. and an amount of Rs. 301312/- has been paid to the Labourers.
After receiving a petition from Bramhanand Sunani PSM of Darlipada GP (Complainant) on Dated 10.02.2018 regarding release of false payment made to 25 labourers who were not at all worked in the aforesaid project at Hatisara which was registered by the undersigned on the same day vide Regd. No 26 which has brought to the notice of the PD, DRDA.
In this enquiry in presence of Gaon SathyChemulal Ahir, PSM Bamanand Sunani, Ward Members of all wards Bargobihari Majhi, Bugal Ahir, AEE Prasant Dharua, J.E Sobhagya Behera, A.P.O Minalal Chandrakar, PEO Pabitra Sahu, Sarpanch representative Sri Rupdhar Deep which Based on the petition, it was noticed that 25 Nos. of job card holders were engaged whom have not worked in the aforesaid project and an amount of Rs 84,480.00 (Rupees Eighty four thousand four hundred and eighty only) which was released through FTO on behalf of the labourers. List of all labourers with respect to the payment made.

Attend Nuapada Block level Mahatma Gandhi NREGS Grievance redressal Camps under MGNREGA schemes. Attended social audit at block level. There are many cases where dispute raised in the public hearing for nonpayment of 95 man days in several GPs of Nuapada District that no automatic Code is generated to allow release of 95 Man days payments to whom RH scheme is given. This is also in connection for the complaints receiving from all Block Level Social Audits conducted recently in Nuapada, Komna, Boden, Sinapali & Khariar Block, that the payment of 95 Man days of Rural Housing Beneficiaries throughout Nuapada District could not be done for which the undersigned conducted a preliminary enquiry and came to know from the PEOs/GRS for the problem, which is due to non generation of Auto Code by the web portal although they have allotted the RH Scheme.

Visited Poir GP after getting complaints in Block level Social Audit. There are two major works executed in the last financial year were grievance related to nonpayment of wages was arised for the work Improvement of road from Poir to Bijay Gad and Improvement of Road from Poir to Anupgad. Personally visited both the works with the APO, PEO, Sarpanch, GRS and the local representative. The work Poir to Bijaygad costing 5 Lakhs is not sufficient to met completion of work as the village Bijaygad is situated in the top platue of Hill and the passage way wash out in Rain. The work of spreading Murum in the slope area is found not feasible and asked to modify the budget as per requirement of Needs. Interacted with the villagers and discussed with their problems related to MGNREGS and suggested many ways how to improve providing of Work, Prompt billing and release of payment. Informed the help line number to everyone. Collected many grievances cross examined with the case records and instructed the GP officials to prompt settle the same and comply.
Dated 17.04.2018 Horticulture- Bhojpur (Khariar Block) Visited Bhojpur village of Khariar for Horticulture Payment and found Dispute to Mango Orchard Beneficiaries.
During the visit of mango plantation taken up under MGNREGA . Since last three years following defects have been noticed which is self explanatory
At Bhojpur GP survival of mango plantation is not satisfactory. Further materials like fertilizer, pesticides etc. has not been provided as per estimates during 1st year & 2nd year of plantation, Udyaan sathy has not been involved and transparency pillar is not found in worksites.
Hence the authorities are directed for taking steps of Gap- filling supply of in plants involving Udyaan sathy & beneficiaries constructed transparency pillar etc. within 15 days otherwise you shall be held responsible for failure of the scheme due to non monitoring & guidance needed for plantation programme.

Visited Saliha for an enquiry for the workers whom not paid wages sit in front of Collectorate Nuapada with the moral support of Garib Sena leaded by Sri Rabi Mangraj. In this connection after consultation with the ADM Sri Minz, as per his advice, the GRS Digambar Lal Naik was questioned and collected wage measurement register and with the help of case record, the pending wages of 4 workers are found which is directed the BDO Nuapada for immediate payment.

Spot visited Biromal G.P of Nuapada Block. There are several agitation from public received for fake payment and non payment of wages in community works, Land Development as well as IAY works and two Fake job card during the visit for which case records for the same was seized by in presence of APO Nuapada, Block Vice Chairman, PEO Biromal .
The case records are Improvement of Road from Darli Nuapada to Mudhela, Improvement of Road from Darlinauapada to Bhandarpuri, Land Development of Ballu Durga and Pinjara Bai Majhi, and 7 PMAY case records of rajni Sahu, Gokul Ram Patel, Khelebai Majhi, Nanukram Rout, Chamru Bariha, Uma Rout. Job card of Bhanat Ram Sahu .

On receipt of grievance petition recorded during social audit at Sinapali block level on dated 05.04.2018. I had visited the work and interacted with the labourers at Liad village under Makhapadar G.P and found that a huge payment of wages to labourers are lying pending since 1 years due to negligence of concerned PEO and GRS of Sinapali block, It is a matter of regret that the concerned PEO is absent and the GRS could not able to produce any relevant records of “improvement of Road Colony Pada” taken up under MGNREGS. The APO is advised to verify the actual status comparing with the case records and measurement of GS to regularize release of pending payments.

Spot visited made to Beheradihi Village of Budhipali GP of Nuapada Block for enquiry of workers engaged in MGNREGS work. Seen projects like Improvement of Road from Dadar Basti to Jenjera, and Palsabhadar to Jonk River, Renovation of Tank at Budhipali. Enquired with the labourers and found that some of the payments are rejected due to inactive Adhar, Account mistake and IIN defects. GS of Budhipali was not been paid her wages which was complained during visit.

Spot visited conducted at Mundapala G.P of Komna Block.
Visited Work of “Imp. of road from Suabahal RD Road to Kendupada(Work code: 2409299)”. Case record of the same was found missing from GP where a huge amount of payment is lying pending. Lot of correspondence was been made for detection of case record since 6 months by the undersigned but failed to find so. Hence you are requested you to prepare a duplicate case record and finailise the pending payments immediately within 15, List of pending measurement annexed here with which was been cross examined and verified by the undersigned in the presence of Komna APO Komna among the workers at respective villages on dated 12.09.2018.
For the work of “Renovation of Kadalipada Khaliabandh Tank”, the labourers have not received their wages as per works done. Payments of wage “Renovation of Palsamunda Tank at Chanamunda” was not paid completely. As against billed amount of Rs. 3,45,356/-, an amount of Rs. 1,51,294 has only been paid. The rest amount is lying pending till.
It was found in all case records that the labour cess amount quoted in all the bills were not paid to respective head. As enquired from the PEO, it came to know that Labour Cess of 1% which was not deducted in any of the case records by any of the PEOs engaged earlier in this GP. Wages of Gaon Sathies not been paid in any of the work. As it came to my knowledge, most of the works, the respective Gaon Sathies were not engaged by the PEOs and GRS for maintaining monopoly.
Following financial misappropriation has been noticed on the work of “Imp. of Road from Bangomunda to Ganjadabar. 1st Running bill prepared for 822 cm Murrum @ Rs 206.40 for an amount of 169661.00 whereas vendor Sri Durjodhan Majhi was paid Rs 194013/- instead of the billing amount, vide FTO No 2428002-ML-1676 dated 25.03.2016 & 2428002-ML-1677 dated 26.03.2016. An amount of Rs 26049/- (including cess deduction) was been paid excess to the vendor for the aforesaid work which must be deducted from the executants.
Cost of labour towards spreading of moorum for the work for the aforesaid work i.e. “Improvement of Road from Bangomunda to Ganjadabar” not yet done, although spreading works has been completed since one year.
Most of the case records are not closed even if the work was completed couple of years earlier. Work site facilities are not provided as per Rules which were also enquired among the workers who were engaged in the aforesaid work. Project initiation meeting was not conducted in the aforesaid works.

Visited Khariar Block for attend Block level Mahatma Gandhi NREGS Grievance redressal Camps under MGNREGA schemes. Various issues related all panchyat are discussed and the solution for the same are taken into the planning of Ombudsman in the forthcoming days.
