Sitting organized at for Case Record & Muster Roll verification of works done at Biromal GP of Nuapada Block.
Similarly, verified projects executed in 2016-17and 2017-18 and interacted with the labourers regarding the flow of wages, issue of Job cards, Identification of fake job cards, Issue of work as well as created awareness among the people for MGNREGS and its Act.
The attended public focused various difficulties based on delay wages, irresponsive Panchayat staff, Corruption using fake job card entries etc in front of the panchayat staff which was taken into consideration and issued show cause notice. Some case records were also seized in presence of panchayat staff where fake muster rolls are generated and misappropriation were made.
Dated -10.05.2018 Gram Panchayat-Jampani (Nuapada Block).
Spot visited Jampani G.P of Nuapada Block. Conduct field enquiry of work and payment made to Mahatma Gandhi NREGS workers. Visited works like Improvement of Road from Jampani to Rohen pani. During the case record inspection in Gram panchayat, several labourers named Ramlal Sahu, Ratan Sahu, Prem Majhi, Jalsingh Majhi, Govind Majhi, Sridhar Majhi, Tikelal Bhoi, Lobind majhi Purmi Majhi, Bhatrat Majhi, Bhoksing majhi and several others demanded release of pending wages through a grievance petition submitted to me as they have meet several times in collector grievance but of no use.
The PEO and GRS were instructed to settle the case at an earliest and the pending wages are assumed Rs 78000/- for the aforesaid work.
Dated -23.05.2018 Gram Panchayat-Kuliabandha (Nuapada Block).
Visited Kuliabandha, Village of Nuapada Block. Recently a payment of wages to the labourers of Adibandh Tank of Gairendihi costing of 2,72,000/- was released by the pancahayat officials through FTO after the strict intervention of ombudsman. A gathering of nearly hundreds of unskilled workers gathered at Gairendihi Village welcoming and greeting the Ombudsman as there pending wages since 1 year released on behalf of grievance petition served to the Ombudsman while on the visit at Kuliabandha GP. The labourers Deonarayan Rout, Lalsing Rout, Sobharam Rout, Anatram Rout, Sabitri Sabar, Toran bai Rout, Sumitra roaut, Kanti Bai Rout, Mili Bai Raut, Hemin bai Rout, Kusmati Harijan and several others were awared about the schemes of MGNREGS and the rights and entitlements for demand of work and demand of wages after work was elaborated in presence of Panchayat and PRI authorities of Kuliabandha GP.
Dated -25.05.2018 Gram Panchayat-Beltukuri (Nuapada Block).
Conducted field enquiry of payment made to Mahatma Gandhi NREGS Workers at Beltukri GP. Received several complaints of Manipulation and misappropriation of Improvement of Road from Pandripani to Singpur where material cost is fed 100 percent and the work was not executed. Improvement of Road from Singpur to Kandhadhap where Rs 2,5,0000/- Two Lakhs fifty thousand was spend without any Muster Roll. The payments were not been made to any labourers as the labour of Singpur village claimed that both the works were not performed by any of the Panchayat Officials.

Visited Kureswar, Reng, Dohelpada under Kureswar GP of Komna Block.Conduct field enquiry of payment made to Mahatma Gandhi NREGS Workers.
Visited works like
Renovation of Kharsel pada Darlibandh at Kureswar, Kureswar G.P Renovation of Pilamunda Tank, at Kureswar MPFP of Sri Rabi Majhi at Kureswar. Renovation of Padarmunda Tank at Kureswar, Excavation of Multipurpose Tank of Rupchand Majhi. Interacted with the labourers Sitaram bag, Khirmohan Sagaria, Ratha Majhi, Ekka hans, Bidyadhar Sagaria Rupchand Ada, Sobha Sing Hans, Dhableshwar tandi, Surat Roan and several others regarding implementation and payment procedure of MGNREGA Scheme. Help line numbers are circulated among all and the PEO and the GRS were instructed to write Helpline numbers in the wall of Gram panchayat for easy disposal of grievance.
Dated -11.06.2018 Gram Panchayat-Kureswar (Komna Block).
Visited Kureswar G.P of Komna Block again as villages like Ghotsar, Dohelpada and Khairbhadi was not able to visit.

Dated -13.06.2018 Gram Panchayat-Thikpali (Komna Block).
Sitting at Thikpali for Case Record & bill verification of LD cases of Thikpali GP under Komna Block. Visited Land development works of Pabitra Sabar, Bisikesan Hati, Dayanidhi Sabar, Jinha Tandi, Beda bag, Tankadhaer sabar, Tarun Das and several others, Physically measurement was taken with the New Ps staffs and JE for settlement of old 1 year pending cases of 72 Land development beneficiaries.
Instructed the Panchayat staffs to cross examine works of all 72 beneficiaries and ensure whether the works are performed manually by them or not before releasing payment.
Dated -15.06.2018 Gram Panchayat-Sunari Sikuan (Khariar Block).
Spot visit Sunari Sikuan G.P of Khariar Block. Verified works like Improvement of Road from Sunari sikua to Gandabahali where it was come to the knowledge that an amount of 32496/- for 21 labourers were not paid by the panchayat authorities due to attaining corruption, the case records are seized by the vigilance department. In such case the PEO and the GRS are directed to pay the balance amount anyhow in Bank account within 1 month otherwise serious action will be taken against them. There after visited the New GP building where also came to know from the huge grievance of public that the labour wages as well as material cost were not been paid due to carelessness of the GP officials. Nearly Rs 44954/- amount of labour payment of 27 Labourers was projected before me by the panchayat authorities to pay shortly. Interacted with the labourers Motilal Bhoi, Santilata bhoi, Girija Bhoi, Luliram Bhoi, Kesav Bhoi, Lata Sagaria, Mukta sabar , Bhairab Charan Das and several others.

Conducted field enquiry of work and payment made to Mahatma Gandhi NREGS workers.Visited works like Compost pit of Lombodhar Majhi, Imp. of Road from Kating pani Main Road to Bijh Darha Tank, Const of Nadep compost of Puna majhi, Const. of Check Dam Bodapani Nala at Chhot Lodra, Ren. of Thuthimauli Arkata at Lodra.
Dated -26.06.2018 Forest Range- Sinapali (Sinapali Block)
Visited Sinapali Forest range and inspection of projects undertaken in the past years and payments disbursed to labourers engaged.
During field visit to all Ranges, the following defects have been noticed during execution of MGNREGA works by Forest Department for the projects executed in the year 2015-16, 2016-17 and 2017-18.
The executing Agency are to provide worksite facility like medical aid First Aid box, Creche, Drinking Water Supply & Shade as per MGNREGA guidelines chapter 7.12.1, 7.12.2 & 7.12.3 and all such expenditure should be booked as part of administrative expenditure which was not found in any of your estimate.
Moreover there must be information/ Transparency pillar as per Annexure-1 near worksite as per MGNREGs Rules 7.10.1 (v). On verification, the above facilities are not provided in any of the work sites under any of your Range whereas in estimates, the provisions/facilities are not been incorporated.
Material expenditures are booked in almost in all projects/ Works. Neither any Vouchers/ supporting documents for the incurred expenditure in accordance to projects verified found in any of the case records nor satisfactory reply received from any of the executing officers at the time of verification.
It was also observed that the existing forest guards are responsible for all field level works such as collection of C1, Distribution of C2, and various other works assigned time to time under MGNREGS apart from their departmental works. They are found untrained about the latest parameters and technical methodology of MGNREGS implementing part, for which it is found an un-satisfactory performance in implementation. Hence it is requested you to engage mate under each project/ Work hence after and make provisions inside your estimate for making payment to the mate under semi skilled wages.
There were no provisions seen for watering facility in any of the case records in Lumpsum mode especially in Tanker rate. All quantities for the same are mentioned in Man-days which is not practically viable in work sites particularly in Avenue plantations. Rather I would prefer you to prepare estimates in accordance with the field necessities and requirements of project instead of prototype.
Watch and ward is a prime responsibility in any of the forest projects which is not taken into serious consideration. Payments are due for the watchman/s engaged in almost in all projects and now they are discontinue in their duties due to long unsolved claims. Most of The estimate for the projects shows the cost of Watch & ward in Man-days which seems not practically possible as seen in fields. The field functionaries are demanding more and more Job cards, Bank Passbooks to compensate a single payment of watchman allocated for the project which is not legally entitled.
Necessary instructions are provided time to time in consultation with the ACF Khariar about the problem, findings and its modification. Whereas it is observed that no modifications are yet taken by your authority in the ongoing projects for the year 2018-19.

Spot visited at Kermeli G.P of Nuapada Block for field enquiry of work and payment made to Mahatma Gandhi NREGS workers. Visited works like Improvement of Golabandh to Keremeli. Improvement of Road from Tileimal to Kadlipada Road, Imp. of Road Golabandha to Raut pada.
Dated -18.07.2018 Gram Panchayat-Kuliabandha(NuapadaBlock).
Hearing conducted at Dhumabhatta, Botha, Kuliabandha and Dhanora villages of Kuliabandha GP of Nuapada Block for pending wages of 4 past projects and back. It was understood from the hearing that an amount of Rs 3.5 Lakhs are due with the panchayat agencies to pay for the past works initiated in the GPs

Visited Watershed area covered under Dhorlamunda GP of Komna Block. Inspection of projects undertaken in the past years and disbursement of payments. Visited field bunding projects at Dhorlamunda of 60 Hc. Agren Goijore Mango Plantation of 6 Hc.
Attend SEW and Watershed committee member in the spot. Discussed with Raghumani majhi, Sobha Majhi, Purno Majhi, Beragai majhi, Ram Rout and several others.

Seen projects like Keshrajpur Avenue Plantation and discussed with labourers named Durjodhan majhi, Raman Majhi, Dhansingh Hathi, Gopo Hathi, Upendra Hathi and several others
Dated -28.07.2018 Hearing report of forest Range-Khariar Road (Nuapada Block)
As per the version from both parties appeared before me on dated 28/07/2018 for the case of Sri Radheshyam Patel, Ramchandra Majhi, Lalit Patel is Khariar Road forest range for a complaint filed on 22/12/2017 regarding nonpayment of water supply charges for the work Masankunda to Bisora road Avenue Plantation “Work Code-2428001/DP/3116530.
Heard both the parties and came to know from them statement that an amount of Rs 65,000/- was settled mutually for the watering expenditure meet from January 15/2017 to June 9/2017 for a period of 6 months through a tanker owned by Lalit Sahu & Ram Chandra Majhi and tractor owned by Radhe shyam Patel who is the leading representatives Kodomeri Village of Kodomeri Panchayat.
On the day of 28/07/2018 called both the parties for a Amicable settlement leaded by Foresters Sri Sadanand Mitra, Sri Tribikram Naik, Forest Guard Sri Tarun Teji, Sri lochan Patel, decided to hand over Rs 30,000 from their side due to accepting their due negligence now the officers posted in Sinapali Range and rest payment will be made from Khariar Road Range by the preset officers Sri Majhi Ranger and Sri Barik Forester for the said work as the work is in 3rd year maintenance stage.
They are directed to release the balance payment of Rs 65000/- Rupees Sixty-five thousand only, as committed by them for water supply for the said work within 15 days from the receipt of this order.
Again it was concluded that a negligence was found execution the said work by the then Ranger Sri Basudev Adbong, Sri Forestor Tribikram Naik, Forest Guard Sri Tarun Teji held responsible for nonpayment of water supply charges at the time of execution of work at Bisora which was settled today.

Spot visit to Siallati G.P of Komna Block. Conducted field enquiry of work and payment made to Mahatma Gandhi NREGS workers.
As per the grievance received at Block level public hearing conducted on dated 12/12/2018 at Komna, The undersigned organized a hearing with the complainants and concerned GP officials in the subsequent day, enquired the matter and found that, payments for the following works are not been paid as per measurement and GS records. List of labourers with respective work is given below.
As a huge amount is pending since years, no action has been taken place even if the complaints were submitted to the BDO as well as the collector Nuapada several times as informed by the labourers. A copy forwarded to PEO Siallati,
He is directed to release the pending payment as per list above comparing with official records of concerned Gaon Sathy, with immediate effect and asked to comply within 7 days.

Visit to Sinapali Forest range and inspection of projects undertaken in the past years and payments disbursed to labourers engaged. Visited works like ANR with GAP Plantation 30 Hc NAGESH A PRF Patel Pada. Permanent Nursery Sardhapur for Convergence in SP Scheme with MGNREGS 5000 Seedlings. Works are performed satisfactorily. No payment related complaints arised during the visit.

Visited Jadamunda G.P of Komna Block. Seen works like Excavation of Dug Well of Sri Sushil Majhi at Thongo and seen some community projects instituted in the last two years, Met the labourers and came to know that the MGNREGS works are not provided in time.
Dated -23.08.18 Gram Panchayat- Michapali G.P (Komna Block)
Surprise visit to Michapali G.P of Komna Block. Conduct field enquiry of work and payment made to Mahatma Gandhi NREGS workers. Visited Works like Renovation of Panchayat Tank at Bishibahal, Reno. of Khaliabandh at Michhapali, Construction of Nadeep Compost of Mangan Majhi Bisibahal, Improvement of Road GP chhak to Gananbahali, Bisibahal Panchyat Tank etc. Sarpanch Ratnadei Majhi, PEO Tekchand Sahu, GRS Simanchal Sabar and JE/GPTA Bhagban Sabar were present.
It also came to know that the IAY 95 Man-days for almost all persons were not received their wages till. As enquired the matter and found that 2016-17-29 Houses, 2017-18-12 Houses, 2018-19 29 Houses completed. Instructed the PEO to release the pending payments immediately. GS Sunil kumar Sahu, Khymasagar Bariha and Pandu Duriya were present at GP. It was also found a wage liability of Rupees One lakhs which was not settled yet.

Verified several files of LD cases are pending with the GPs. Beneficiaries named Arsing Hans, Lakhman Bandichor and 14 others and 6 beneficiaries Kunjan Dansena, Loknath Dansena and others for Compost pit payment were also not made. In addition to that, nearly 40 beneficiaries of 2016-17 were also not been paid the IAY MGNREGA share. Similarly Bhubaneshwar Kharsel and Tirtha Kharsel were not paid the IAY MGNREGA Share due to non generation of Auto code.Debadatta Hans and Nile Chatria were not paid their Mate charges of Rs 8406/- which was instructed to pay immediately.

Verified various case records like Improvement of Road from Bangomunda to Ganjadabor where it was found an excess payment of Rs 26049/- was paid to Duryodhan Majhi for supplying of Murum. Improvement of road from chanamunda to jharia pada and Renovation of Bongomunda Mondai tank where cost of Labour cess and GS payment were not paid.
Kadlipada Khalia Tank file and bill for the same was missing as the labourers engaged in the said work has complained several times for release of their pending dues. In this context, a joint verification was conducted with the APO, GRS, PEO among the villagers where the quantity of work done was conformed as per actual in field status in accordance with the measurement made through the GS register and advised to release the payment at an earliest.
Dated -20.09.2018 Gram Panchayat-Poinr (Komna Block)
Visited Poinr G.P of Komna Block. Conducted field enquiry of work and payment made to Mahatma Gandhi NREGS workers of Imp of Road from Poinr to Bijaygarh vide project code No 2428002029/RC/3101299 issued in the year 2015-16 costing Rs 5 Lakhs. The 55 job card holders were worked with their team/family members in the aforesaid work. The case was registered as the final/ completion bill was measured by the GS Loknath Majhi which has not taken into consideration by the JE so further Muster Roll was not prepared and payment was not made accordingly.
Meanwhile the complaint was attend after getting several complaints from the villagers who attend at Block level public hearing organized at Komna block, where in the same complaint was also encouraged by the electronic media O-TV at the spot, and from an enquiry, it came to know that the said project was executed in the aforesaid year, The case record and other relevant documents such as Measurement Book was produced by the present P.E.O Miss. Sasmita Behera.
It was understood from the grievance submitted by the labourers through the petition, that a quantity of 2165 ft3 pit was digged by the complainants which was cross examined with the measurement records with the GRS that an average quantity of 2090 pit of work was digged by the labourers since date which is estimated approximately Rs 7,35,715/- @ Rs 174/- per day, by the APO Komna through his written statement submitted during the inspection date which is more than the approved estimated cost of the said project. Further due to non availability of other supporting documents the GRS was to submit the exact information within 2 days meanwhile the quantity of work indicated through the petition listed as follows.
In view of the above situation, the APO Komna was asked to prepare a Work-done estimate with the assistance of JE Bhagban Sabar to release the pending amount of wages which exceeds the approved estimate.

Visited Bhuliasikuan G.P of Khariar Block and Conducted field enquiry of work and payment made to Mahatma Gandhi NREGS workers.
Seen works like Banchormunda Tank work at Bhulia sikua, Arkata Tank, Imrovement of chatiaguda to chitadungri road, Improvement of Road from Chhatta to chutigudi , formation of Road from chhatta canal road to Chutigudi Road.
No complaints were received from this GP regarding for any pending payments.
Dated -04.10.18 Gram Panchayat-Khutban Bhera G.P (Nuapada Block)
Visited KhutbanBhera G.P of Nuapada Block for an enquiry regarding work and payment made to Mahatma Gandhi NREGS workers of last 2 financial years. Visited works like Renovation of Narmada Tank at Panchampur and Improvement of Tipajhar Tank at Tipajhar. Interacted with labourers Pilu Paharia, Alekh Sabar, Digamber Misra and several others and found no dispute of payment. Although the payments are received too late, the labourers used to manage themselves adding more work in chain so payments of earlier work is getting paid. Workers demanded more work this year as the panchayat officials are not taking interested in implementing new projects.

Visited Darlimunda G.P of Nuapada Block. Seen works like Renovation of Mundabandh Tank and Renovation of Badamunda Tank at Anlajuba intracted with laborers Bira Majhi,Dingar Majhi,Faguni Majhi,Tikalal Majhi and others. No complains of any pending payments with respect to works implemented in the past year.

Visited Tarbod G.P of Komna Block. Conducted field enquiry of work and payment made to Mahatma Gandhi NREGS workers. As against the bill amount passed for Rs.307868/- amount paid for Rs.291456/- only leaving balance payment for Rs,16412/- .Advised PEO to pay the same immediately like the same Second running bill Passed for Rs.210841/- but paid 216128/-. As such excess amount paid for Rs.5314/-. And for 3rd running bill passed for payment made for Rs.66850/- but Payment made for Rs.27982/- only, For 4 th running bill of Rs.221208/- no payment made so far. Advised PEO and GRS for payment of entire amount for which Passed for Payment done immediately.It was observed that without recording Pre-measurement of tank for renovation Plan, estimates and bills have been prepared which is illegal and the entire amount is recoverable from the concerned employees.

Ist year maintenance of Plantation from Sardhapur to Karlapani and Avenue Plantation Kirkita to Sunarisikuan ocovering 6 k.ms. Advised Range Officer to pay the Nursery cost immediately.
